Funeral services will be conducted 2:00 p.m. Saturday in the chapel of Edmaiston-Mosley Funeral Home. Burial will follow in Jenkins Cemetery near Martin.

The family will receive friends at Edmaiston-Mosley Funeral Home from 12:00 p.m. until the hour of services Saturday.

Pallbearers will be Jody Hazlewood, Steven Shanklin, Matthew Hazlewood, James Rhamy, Lester Brundige and Pete Cook.

Mr. Cook was born November 19, 1959, in Hickman, Kentucky, son of the late Earnest Talmadge and Wilma Fern (Childers) Cook. He married the former Tammy Oseman on May 2, 1981. She survives. He had been employed as a welder for Trevathan Brothers in Dresden.

He is also survived by two daughters, Deanna Couch of Greenfield and Melissa Willcutt of Union City; four sisters, Brenda Hazlewood of Martin, Barbara Hazlewood of the Sidonia community, Debbie Asher of Union City and Donna Eubank of Greenfield; his brother, Larry Joe Cook of Martin; and six grandchildren, Cameron Couch, Bently Couch and Eason Couch all of Greenfield and Conner Blaze Willcutt, Logan Willcutt and Jaden Willcutt all of Union City.
