Leo Conrad "Guy" Forrest, 49, of Union City passed away 10:33 a.m. Wednesday, September 14, 2016, at Jackson Madison County General Hospital.
Funeral services will be conducted 2:00 p.m. Saturday in the chapel of Edmaiston Mosley Funeral Home. Bobby Stewart and Father Robert Ponticello will officiate. Burial will follow in Antioch Cemetery near Hornbeak.
Visitation will be held at Edmaiston Mosley Funeral Home from 11:00 a.m. until the hour of services Saturday.
Pallbearers will be Aaron Mudd, Jason Adams, John Thompson, Terry King, Jimmy Vowell, David Hutchinson, Daniel Gooch and Terry Copeland.
Memorial contributions may take the form of donations to
St. Jude Children's Research Hospital
or
The Wounded Warrior Project.
Leo was born September 16, 1966, in Denver, Colorado, son of the late Leo Conrad His Bluehorse and Myrna (Whitehawk) His Bluehorse. He married Diane (Yochum) Forrest September 7, 2003. She survives. Leo was employed at MIA Seating Company in Union City and had been employed at the former Lennox Hearth Products for 22 years. He served in the United States Army 82nd Air Borne Division and was a member of the Oglala Lakota Sioux Indian Tribe out of Rosebud, South Dakota.
He is survived by his adoptive parents, Patsy Forrest of Hornbeak and Roger Dale Forrest of Troy; one daughter, Jessica Marie Hobbs of Union City; one son, Joseph Conrad Forrest of Nevada; two stepdaughters, Dana Marie (Casey) Bell of Martin and Debra De Lise Davis of Jackson; one stepson, Daniel Parker Passmore of Martin; three grandchildren, Abby Brooke Davis of Jackson, Alexis Danielle Forrest of Union City and Drake Colin Bell of Martin; one sister, Michelle Golightly of Wingo, Kentucky; three brothers, Forrest Bluehorse of Rapid City, South Dakota, Troy Forrest of Troy and Lee Simmons of Jackson; his father-in-law and mother-in-law, Charles and Dorothy Yochum of Union City; his aunts, Ilene Hutchison of Hornbeak and Bonnie Siler of Germantown; and several other uncles, aunts, nephews and nieces.
He was also preceded in death by one sister, Connie Bluehorse; and his grandparents, Jess W. "Billy" and Lou Annie Burton.
