Cathy Connell Hazlewood, 52, of Martin passed away 11:40 p.m. Wednesday, July 28, 2021 at her home.

Visitation will be held at Edmaiston Mosley Funeral Home from 2:00 p.m. until 3:00 p.m. Saturday.

In lieu of flowers memorials can be made to her husband Chad Hazlewood or to St. Jude Children's Research Hospital, 501 St. Jude Place, Memphis, Tennessee 38105-9959

Cathy was born August 3, 1968, in Fulton, Kentucky, daughter of Larry and Janice (Rothman) Connell of Fulton, Kentucky. She married Chad Hazlewood December 22, 2000. He survives. Cathy had been employed as a LPN with Adoration Health.

She is also survived by two daughters, Sydney Hazlewood and Emma Hazlewood both of Martin; two sons, Hunter Choate and Ethan Hazlewood (Kim) all of Martin; two grandchildren, Caleb Choate of South Fulton and Sophie Choate of Union City; two sisters, Donna Dean (David) of Danville, Virginia and Susan Connell of Union City; four nieces; and two nephews.

The family would like to thank Cathy's nursing friends and patients and their families for their care and support during Cathy's illness.
